The “My EAMC” patient portal is a tool available to EAMC patients who wish to view their medication list, allergy list, patient education documents, and some lab results after leaving the hospital. This portal will allow patients (or caregivers, when appropriate) to be active participants in their healthcare plan.

In order to participate, all we need from you is your email address so we can provide you with an invitation to use the portal. Once you receive your invitation, you will go through a couple of steps to get registered and then you can view your information from the comfort of your personal computer.

Your steps to connecting to the “My EAMC” Patient Portal:

  1. Provide the registration staff with your email address when you are getting registered
  2. You will receive an email from noreply@iqhealth.com inviting you to get connected with your medical information by registering for My EAMC. (Check your junk or SPAM folder if you do not find it.)
  3. Click on the link in the email
  4. You will be asked personal information to confirm your identity
  5. If you weren’t able to give your email address at registration, you can give it to your nurse during your stay to get your email invitation sent
  6. If you did not provide your email address during your stay or you haven’t had a recent visit at the hospital, please print and complete the authorization form in the link below.
  7. Provide a copy of your photo ID and the completed form to the medical records department. Once this form is signed and approved, you will receive an invitation to your personal e-mail to set up your user name and password for the patient portal.
