East Alabama Medical Center’s Skilled Nursing Facility offers a transitional level of care to those patients needing rehabilitative and restorative services on a short-term basis. The Skilled Nursing Facility is most appropriate for patients who are not ill enough to be in an acute care hospital but are too ill to return home or to be maintained in a traditional nursing home.
Services provided include:
- Nursing care and management
- Dietary and nutritional services
- Physical, occupational and speech therapy
- Respiratory therapy
- A coordinated and planned activities program
- Social services
- Team approach to discharge planning
The location within the hospital allows physicians better access to the patients than facilities outside of the hospital. All of East Alabama Medical Center’s services are available to the Skilled Nursing Facility patients just as they are to other hospital patients 24 hours a day, seven days a week.
Admission to the Skilled Nursing Facility is carefully regulated by state and federal agencies.
To qualify for admission to the facility, a patient must:
A. Have been in a hospital for 72 hours within the past 30 days, and B. require skilled daily services of a licensed nurse or therapist.
The patient’s physician must certify that the skilled services are needed and are related to the condition for which the patient was hospitalized. The physician ordering the admission must be on East Alabama Medical Center’s list of credentialed physicians. For more information, call 334-528-4172.
