Being safe about your medications could save your life. One important safety tip is to know exactly what medications you take, how much you take and when you take them ... and to be able to give this information to your caregivers.
Print the universal medication form (link below) and fill it out, then keep it in your wallet or purse. Be sure to update it whenever you add or discontinue a medication. This will help ensure that your healthcare providers are aware of your medications and how that impacts your care.
